Position / Responsibilities

Spectrum is looking to hire an innovative leader to join the organization as Program Director, Integrated Care. Under the guidance of the Regional Director, and in collaboration with the Corporate Integrated Care and Partnerships and Clinical Quality Teams, the Program Director, Integrated Care, will support the development and lead the implementation of multiple Integrated Care Programs that uniquely focus on co-design with system partners (i.e. Hospital to Home). Reporting to the Regional Director, you will lead the local implementation and on-going operations of these programs.

As a champion for positive health system change and outcomes, you will develop and maintain key relationships with clinical and operational health system partners to contribute to the implementation and sustainability of integrated care programs. Leveraging a One Team approach, you will lead the operational planning and implementation of multiple concurrent integrated care programs, including defining service delivery requirements locally in collaboration with various external partners and patient populations.

Requirements

The Program Director, Integrated Care, will have a degree in a relevant health-related field and be designated as a Regulated Health Professional and a member in good standing with the relevant professional college or association. With 5-7 years of leadership and management experience in complex healthcare organizations, the Program Director, Integrated Care will have a proven track record in healthcare operations, change management and program development with multiple stakeholders in complex environments, and a strong knowledge of Ontario’s health system design, funding models and an understanding of priority issues facing local/regional health systems.

As a team player with a results-oriented approach and a motivated self-starter with a successful track record of building partnerships, you will build a culture of collaboration, excellence and continuous improvement. As a champion for excellence in patient and client-centred care for all, you will promote and demonstrate an understanding of working in a culturally diverse and changing community environment.
